Services We Provide:

J&M Community Services provides Individualized Skills and Socialization services to eligible individuals participating in Texas Medicaid waiver programs, including:

- Home and Community-based Services (HCS)

- Texas Home Living (TxHmL)

- Deaf Blind with Multiple Disabilities (DBMD)

These programs help individuals with intellectual and developmental disabilities receive essential services and supports while living and participating in their communities.

We work closely with each individual, their family or legally authorized representative, and their Medicaid waiver program provider to ensure our services support the goals and objectives identified in the individual’s approved service plan. Our person-centered programs focus on independence, skill development, social interaction, personal choice, and meaningful community participation.

Participation and services are based on Medicaid eligibility, program enrollment, individual needs, and authorization from the individual’s waiver program provider.

Private Pay Options:

We offer private-pay options for individuals and families seeking structured day activities, independent living skill development, socialization, recreation, and meaningful community participation.

Private pay may be an option for individuals who are not enrolled in a Texas Medicaid waiver program, are waiting for waiver services, or would like additional services beyond those currently authorized. Our team works with each individual and family to develop a schedule and service plan that reflects the person’s needs, interests, abilities, and goals.

Services are provided under a written private-pay agreement that explains the approved schedule, rates, payment requirements, and responsibilities of each party. Availability and pricing may vary based on the services requested and the level of support needed.
